Position Overview:
Join a dedicated care team designed to provide patients with exceptional, personalized care in an outpatient setting. Our collaborative team includes Physicians, Advanced Practice Professionals, Nurses, Social Workers, Case Managers, Care Coordinators, Behavioral Health Support, quality-focused Coders, Referral Coordinators, and more. This integrated approach allows our primary care physicians to focus on quality patient interactions and foster a holistic care experience.
Primary Responsibilities:
- Conduct comprehensive evaluations and manage patient care per established standards.
- Coordinate additional services as needed, including specialty referrals, durable medical equipment, hospital and SNF transitions, and home health care.
- Serve as a key clinical advisor and active participant in daily team huddles, contributing medical insights to support optimal patient care.
- Partner with the Lead Physician and Center Administrator to promote a culture of collaboration and professionalism within the practice.
- Complete and optimize medical documentation daily with the assistance of quality-based coders.
- Adhere to clinical protocols and policies set by Clinical Leadership.
- Regularly consult with the Lead Physician on quality of care, outcomes, and documentation standards.
- Dedicate focus to patient care, including both patient-facing time and related administrative activities.
Required Qualifications:
- MD or DO degree from an accredited medical school.
- Unrestricted medical license in Arkansas (or willingness to obtain), with potential to obtain additional state licenses as needed.
- Board Certification or Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine (ABMS or AOA preferred).
- Active and unrestricted DEA license.
- Medicare and Medicaid Provider Numbers.
- Strong interpersonal skills for effective communication with patients and colleagues.
- Commitment to a team-based, value-driven care model.
- High patient and customer service standards.
- Basic proficiency with computers, email, and electronic medical records (EMR).
Preferred Qualifications:
- Familiarity with HEDIS/STARS metrics.
- Knowledge of CAHPS and HOS performance measures.
- Experience with electronic health record systems.
- Background in serving medically underserved or culturally diverse communities.
- Previous experience with Medicare Advantage Global Risk Models.
