Job Title: PCP/QMAP Shift Manager
- Full-Time Or Part-Time-1 Manager per shift.
- Day shift.
- Every other weekend required.
- Rotation of Holidays required.
Location: 900 Centre Ave. Fort Collins, Colorado.

About the Role:
- As a PCP/QMAP Shift Managerat New Mercer Commons, you will:
- Provide direct care to assigned residents, assist with medication administration, and ensure residents' belongings (e.g., hearing aids, glasses) are accounted for by the end of the shift.
- Communicate resident status changes to the LPN/RN and ensure all required documentation is completed.
- Coordinate breaks for staff, respond to calls and pages, and welcome new residents to the facility.
- Attend required meetings and trainings andprovide orientation and training tonew staff members.
What We're Looking For:
- 1+ year of experience as a care provider required.
- Completion of 16-hr. Personal Care Provider training offered through CHS required.
- Qualified Medidation Administration Professional required.
- CPR certification required.
- Must be at least 17 years of age.
- Interest in geriatric population preferred.

- Leadership and Team Coordination: Ability to lead and coordinate a team, manage schedules, and ensure smooth workflow during shifts.
- Communication Skills: Proficiency in effectively communicating with staff, residents, and families, as well as relaying critical information to nursing staff.
- Clinical and Direct Care Skills: Competence in providing hands-on care to residents and managing medical tasks such as medication administration.
- Problem-Solving and Decision-Making: Ability to address issues promptly, make quick decisions, and resolve conflicts as they arise.
- Documentation and Record-Keeping: Accuracy in maintaining records, ensuring compliance with documentation requirements, and tracking resident information.

- Dependability: Being reliable and consistent in overseeing care, following procedures, and supporting the team.
Our Total Rewards package includes a salary range of between $17 - $22/hr. DOE
