Community Bridges is looking for 1 full-time PDMS Program Coordinator!
We are a non-profit agency founded in 1982 with a mission to advance the integration, growth, and interdependence of people with developmental disabilities within their own communities. Building on the strengths of over 1,100 individuals in the greater Capital Area and their families, we work to identify barriers of access to find creative solutions to develop meaningful community engagement.
The PDMS Program Coordinator is responsible for all day to day operations in the Self-Directed and In Home Support department. The PDMS Program Coordinator will assist in ensuring that He-M 525 and He-M 524 Services, as well as agency policies and procedures are upheld throughout the delivery of services. The Program Coordinator provides a high level of customer service in all interactions both internally and externally. The Program Coordinator is responsible for the handling of all administrative day to day operations by organizing and optimizing processes within the department and in collaboration with other departments.
Qualifications:
Education: Associate's Degree required or equivalent work experience.
Experience: At least2 years' experience working in an educational, health care or human services field. At least two years of general bookkeeping. A high level of administrative skills, attention to detail and meeting deadlines.
Compensation and Benefits
There is a $1000 sign-on bonus for this position - $500 will be paid at the time of hire and $500 will be paid after 6 months in the role. *Please note, sign-on bonuses are for new hires only. Re-hires and transfers are not eligible at this time.

Specific Duties and Responsibilities:
- Provides ongoing support, problem solving, and mentoring for team members and interacting with other departments as relates to the agency, the mission, and the program specifics for self-directed and In Home Support Services.
- Maintains a professional atmosphere in the department and represents the department in both agency and external meetings meeting a high level of customer services standards.
- Continuously looks for ways to improve process and procedures that enhance the support and services that the PDMS department provides. Shares and provides ongoing ideas and creativity for enhancement in all administrative areas for the smooth operations of the PDMS Department.
- Understand person centered planning and use those skills in the creation of communication with all new hires, families, and community member interactions.
- Assist with the continued development of department and program policies and procedures.
- Support individual and their family/representative to recruit and supervise family managed employees providing orientation/consultation/training and review of time keeping supervision.
- Provides an exceptional customer service experience working with families to onboard new hires. This includes; coordinating the new hire experience for families, completing new hire paperwork, overview/training for new hires on timesheets and access to work tools, monitoring schedules, review of employee paperwork, answering questions from employees and families, and training needs.
- This role will be the liaison to Human Resources. Working collaboratively with HR to provide completed background check packets, back ground rechecks, updates on status, schedule changes, pay, recruitment ads, performance review tracking, and address changes for system updates as needed.
- Complete all necessary record keeping tasks to document the provision of services to fulfill agency administrative need with regard to certification, record keeping, quality assurance and satisfaction.
- Provide administrative support in tracking of 525 certs., completion of payables, ordering supplies, filing, and the administrative organization of department.
- Maintain involvement in all aspects of the self-directed and In Home Supports services programs such as strategy marketing, implementation and delivery.
- All other duties as assigned.
Required Skills:
- Possess strong verbal and written communication skills. Ability to communicate thoughts and ideas in either group or one to one forum.
- Strong adherence to Community Bridges philosophy, mission and values.
- Working knowledge of agency policies and procedures and applicable state regulations.
- Working knowledge of accounting software as well as proficiency in Word and Excel programs.
- Ability to work independently and with others. Role models professional behavior standards set by the agency.
- Good organizational, time management and problem solving skills.
- Ability to work flexible hours.
- Ability to design presentations.
- Understanding of a variety of funding resources including local, state and federal public health benefits, grant and charitable resources.
- Ability to represent Community Bridges to the public in a manner that promotes a positive public image.

Work Environment and Physical Demands:
Work Environment- occasionally hectic with periods of high stress. Occasional to moderate short deadlines and coverage that requires flexibility in schedule.
Physical Demands- Walking and standing are required frequently with occasional stair climbing. Lifting up to 20lbs on occasion.
Travel:
Travel required within the state occasionally. Must have access to personal vehicle during all scheduled work hours and uphold an acceptable driving record.
