After Hours Weekend RN ~ Friday 5pm to Monday 8am
We are currently recruiting for our hospice's after-hours nursing team. As an after-hours nurse, you are never alone. You work with other members of the after-hours team in coordination of visits to patients who are in need of care outside our normal business hours. Our after-hours team is scheduled on Friday at 5pm until Monday at 8am. During your weekend on, the expectation is simply that you are available if a need arises outside of normal business hours with one of our patients. If there are no needs, you are free to spend your time how you like, as long you are able to commute to a care need in a timely manner.
Responsible for managing all aspects of the patient intake process after hours, including managing the members of the intake team, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.
Essential Job Functions/Responsibilities
· Available to triage and see a patient within the Friday 5pm to Monday 8am schedule.
· Conducts patient visits and gathers information for RN Case Manager.
· Performs treatments, including wound care, as directed by the RN Case Manager.
· Assists in identifying discharge-planning needs.
· Reports changes in the patient’s physical condition or care needs to PCP.
· Orders medication, DME and other supplies to be delivered to patient as needed.
· Participates in quality assessment and performance improvement activities.
· Works with Admissions Coordinator and admissions team to ensure timely evaluation and admission of patients to hospice, when admission is scheduled outside of normal business hours.
· With regard to admissions, ensures seamless transition of patients to hospice care by providing direct oversight of patient education and preparation for hospice care, plan of care initiation, and coordination of care with multiple service providers.
Position Qualifications
· RN with current licensure to practice professional nursing in the state.
· Demonstrates good communications, negotiation, and public relations skills.
· Ability to assess patient’s needs and direct care to meet those needs.
· Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
· Valid Driver’s License.
· Automobile in good working order.
· Proof of current automobile insurance.
