Job Description
Join our team as a per diem Social Worker. In this role, you will be responsible for providing social work services to the residents and family members of Continuing Care.
Compensation: $33-$36/hour
Schedule: Must be available during regular business hours (Monday - Friday)

How you will make an impact
+ Completing admission assessments, MDS, and other required documentation in accordance with State and Federal regulations
+ Educating residents/families of the importance of advance directives and facilitating completion, signature, and tracking of documents
+ Coordinating the Social Work function from admission to discharge to include room changes and transition planning
+ Providing psychosocial support to residents and families to ensure a successful transition to Continuing Care
+ Participating in care/service plan meetings with family members
+ Providing bereavement support to residents, family members, and staff
What you will need
+ Social Work degree (BSW or MSW) required, or minimum education requirements as per state licensure.
+ Minimum of 3 years’ experience in working with the older adult population in post-acute/ long term care preferred.
+ Knowledge of geriatric population and dementia.
