POSITION: Per Diem Registered Nurse Case Manager / Supervisor
SUPERVISOR: Administrator
STATUS: Non-Exempt
POSITION REQUIREMENTS: This position is primarily per diem with the possibility of growth. The Registered Nurse (RN) is responsible for the service provided to the clients of Always Best Care Senior Services, (the Agency), and for supervising the delivery of services, according to the philosophy, standards and all AHCA Accredited Policies and Procedures of the Agency. The RN will ensure that all direct care staff provide safe, appropriate, prompt, courteous and professional service to clients according to the client's Plan of Service. The position will be supervisory to the services provided by all Homemakers/Companions, Home Health Aides, Certified Nursing Assistants, LPNs.
Essential Functions:
Conduct initial assessments of each potential client.
Create a Plan of Service for each client admitted to the Agency.
Provide case management of specifically assigned clients as per Agency policy entitled Case Management.
As additional employee positions are created, or as vacancies occur, assist the Administrator and Operations Manager in recruiting personnel, as needed.
Participate and provide input to the Quality Assurance committee and ensure the quality of services is maintained.
Comply with all federal and state rules, regulations, and laws, with respect to home health care service.
Comply with infection control regulations, aseptic techniques, and OSHA Rules.
Comply with company policies.
Other duties and special projects as assigned.
Supervisory Responsibilities:
Ensure that all legal standards of practice are maintained by all employees providing service on behalf of the Agency.
Supervise, monitor and evaluate employees who provide direct service to Agency clients as needed.
Conduct competency evaluations of direct client service personnel as needed.
Participate in the selection, training, discipline, and termination of all direct client service employees as needed.
Conduct Case Management Functions as needed.
Always retains the full responsibility for the care given to clients under his/her supervision and for making supervisory visits to the client's home.
Be available to train and conduct the orientation of all direct care personnel as per relevant policies and procedures as needed.
Be available for supervision of direct service employees as assigned by the Administrator.
Qualification, Knowledge and Skills:
Clear a Level 2 Criminal Background screening through AHCA, the OIG Exclusion List and a National Sex Offender Registry check.
Be licensed, in good standing, as a Registered Nurse in the State of Florida.
Graduate of a school of professional nursing is required; Baccalaureate degree in nursing preferred.
Minimum of two years of home care experience as a professional nurse is required, with one year of experience in a supervisory position preferred.
Superior organizational, English language communication (verbal and written) and customer service skills.
Ability to interact professionally with clients, physicians, vendors and Agency employees.
Ability to work in a demanding and fast-paced environment.
Fiscal Responsibility: None
Extent of Public Contact:
Considerable personal and telephonic contact with clients, caregivers and client's family members.
Some telephonic contact with physicians.
Physical Demands:
Driving throughout the Agency Geographic Service Area; telephonic contact in addition to individual meetings with clients, clients' families and caregivers; lifting boxes of supplies weighing no more than 10 pounds.
Work Conditions and Environment:
Office environment and in clients' homes.
