Pharmacist On-Call - NEW - Kalihi Palama Health Center

Primary Purpose:

The Pharmacist processes and fills prescriptions, interacts with providers on pharmacotherapy interventions, supervises pharmacy technician, performs day-to-day staff pharmacist functions and participates in pharmacy initiatives as assigned.

This position is an on-call position and will cover when Pharmacist is not in the clinic.

Essential Duties:

• Assume overall responsibility for the operation of the Pharmacy.
• Supervise the upholding and updating of the Health Center Formulary.
• Develop appropriate drug inventories.
• Maintain accurate records of drugs for both internal and external monitoring & control.
• Be responsible for the security of pharmaceuticals.
• Be directly involved in drug dispensing and patient counseling.
• Be responsible for correctness and completeness of all medications dispensed under his/her supervision.
• Develop in service training program for Pharmacy staff.
• Direct training of professional and supportive personnel.
• Order drugs and supplies; responsible for all administrative functions.
• Generates appropriate reports as determined by CEO, CFO, and Medical Director.
• Perform other related job duties as assigned by the supervisor.

• Communicate problems, concerns and suggestions to the medical director and to other adult team providers verbally and in writing where appropriate.
• Maintain current licensure to practice in the state of Hawaii.
• Maintain charting in accordance with legal and clinic standards.

Reliable on-site attendance, when called, is required.

Other Duties:

· Perform other related duties as assigned

• Observe safety and security procedures; determine appropriate actions beyond guidelines; report potentially unsafe conditions; use equipment and materials properly.

Minimum Qualification Requirements:

Education:

· Doctorate of Pharmacy from an accredited university preferred.

· Current State of Hawaii Pharmacist license required.

· Bilingual – English & Chinese and/or Indo Chinese preferred.

Experience:

· Must possess a state of Hawaii Pharmacist license, meet the certification requirements and be a pharmacist in good standing.

· Minimum three years of experience in a community health pharmacy or other pharmacy setting required

· Must be eligible and willing to become credentialed with third-party payers as appropriate

Skills/Knowledge:

· Strong written and verbal communication skills.
