Job Details Pharmacist The Pharmacist follows hospital and departmental policies and procedures regarding the preparation, storage, control and dispensation of inpatient and outpatient medication prescribed by members of the Medical and Dental Staff. Responsibilities: Demonstrates the ability ensure appropriateness of medication orders placed by providers Contacts providers to resolve medication related issues Dispenses prescription medication to inpatients and employees as per departmental policies. Appropriately supervises pharmacy support (e.g., technicians/interns) personnel. Appropriately checks all work conducted by pharmacy support personnel requiring direct pharmacist supervision and authorization Dispenses and maintains accurate records of dispensing controlled substances, tax-free alcohol and investigational new drugs as required per federal and state regulations. Demonstrates ability to operate required pharmacy automation equipment, e.g., unit-dose packager, automated dispensing cabinets Compounds product as required, including sterile parenteral products, maintaining required logs. Initiates and documents pharmacist interventions where necessary. Investigates and documents reports of Adverse Drug Reactions. Ensures an adequate inventory of medications and supplies is maintained. Arranges for emergency inter-hospital transfer of medications when required. Completes all required documentation. Maintains a safe, clean working environment. Maintains and enforces appropriate security measures at all times. Prepares required reports of pharmaceutical services. Provides drug information services to interdisciplinary personnel. Demonstrates ability to retrieve and convey information from various sources Adhere to Department policies and procedures Demonstrate compliance to continuing education requirements as dictated by Department and regulatory needs. Requirements: Education: Graduate from an ACPE-accredited pharmacy program Pharm.D. preferred Experience: Previous hospital pharmacy experience preferred Licenses: Currently, licensed and registered as a Pharmacist in the State of New York Availability requirements: Fulfill at least two full shifts in a four week schedule Salary Range USD $60.90 - USD $86.61 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
