Pharmacist Specialist - Medication Safety and Quality - Cottage Health

The Medication Safety and Quality Pharmacist Specialist is responsible for guiding the design, implementation, maintenance and evaluation of safe medication systems within the health-system. The specialist provides pharmaceutical care to achieve improved outcomes and quality of life for patients. The specialist is responsible for working collaboratively with all members of the healthcare team to ensure that medication safety goals and quality improvement objectives are achieved. The Medication Safety and Quality Specialist develops medication safety initiatives and works independently and collaboratively to investigate, implement and monitor medication safety initiatives, and ensures compliance with medication safety standards and regulations. The specialist serves as a consultant on medication/patient safety and quality standards related to medication management (i.e. TJC, CMS, DPH, NQF, IHI, ISMP, Leapfrog, and FDA standards) to provide ongoing accreditation and regulatory readiness, and evaluates compliance with and assists with the implementation of TJC National Patient Safety Goals and other medication safety and management related requirements. The Medication Safety and Quality Specialist serves as a leader for the department in their specialty area through mentorship, education, and serving as a clinical resource for pharmacy staff, physicians, affiliate providers, nurses, and preceptor for pharmacy residents and students.

PharmD or Bachelor of Pharmacy degree from a college of pharmacy. Completion of a post-graduate residency or fellowship training is preferred. If specialty residency training is available in specialty area then specialty residency is preferred.

Training in areas of clinical pharmacy related to specialty area of practice required.

Current pharmacy licensure in the state of California. Board certification preferred.

Completion of a post-graduate year 2 residency (PGY2) or fellowship or Completion of a post-graduate year 1 (PGY1) plus 3 years of experience as a clinical specialist or equivalent role which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring in the specialty area or 5 years of experience as a clinical specialist or equivalent role which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring in specialty area.
