At Valley Regional Hospital, patients are the center of our coordinated care system and right now we have an opening for a Pharmacist to join our team.
The primary responsibility of the Pharmacist is:
- Review medication in order to provide accuracy and safe medication usage based on current guidelines and best practices as well as patient-specific factors.
- Ensures appropriate action is taken to avoid drug allergies, drug interactions, duplicate therapy and incompatibilities by collaborating with an interdisciplinary team
- It acts as a drug information resource for multidisciplinary team including providers, dietitians, nursing staff, other disciplines and specialty services as necessary.
- Detects, monitors, documents, and reports adverse drug reactions and medication errors.
- Oversees pharmacy personnel, participates in proper techniques and accurate preparation of all pharmaceutical products, including oral, non-sterile, and sterile compounding, chemotherapeutic and hazardous medication following current USP standards and state and federal regulations.
An ideal Candidate would have the following qualifications:
- Doctorate of Pharmacy degree or Bachelor of Science degree or equivalent.
- Graduated from an accredited School of Pharmacy.
- Current Pharmacist certification or licensure in the State of New Hampshire.
- Education in all age groups ranging from newborn to the geriatric population obtained either in school education or subsequent training.
