POSITION SUMMARY
Position is responsible for the implementation and evaluation of community physical activity programs outlined in the Special Diabetes Program for Indians (SDPI) grant. Incumbent will facilitate collaboration and partnerships with local schools, chapters, and public health organizations to improve youth, adult and senior fitness levels. Conduct assessments to determine physical activity needs of community or individuals. Serve as a resource to community and schools on physical activity. Provide additional support to diabetes intervention programs as health coach. Assist with other SDPI activities as it relates to physical activity. Knowledge of local tribal philosophy and cultural practices is a must. Incumbent must be willing to work evenings and weekends, and as needed. This position is grant funded.
Qualifications:NECESSARY QUALIFICATIONS
Education:
Bachelor’s degree in Exercise Science, Kinesiology, or related degree
Experience:
- Must have six (6) months experience in Health Promotion or community activity programs that applies knowledge of principles, methods, and techniques to plan, implement and evaluate health programs.
- Must have experience working with individuals with diabetes, hypertension, heart disease, and others while utilizing innovative ways to motivate and encourage clientele
Certifications:
- Must have and maintain a Basic Life Support Certification (BLS) from the American Heart Association (AHA) with AED certification. Or must obtain within 90 days of hire.
- Training or Certification in Group Fitness or Personal Fitness Must obtain within 120 days of hire
- Must maintain and have an unrestricted current valid state driver’s license

A record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers. All employment references must address and indicate success in each one of the following areas:

PHYSICAL:
Must be able to exercise physical exertion when overseeing activities, including all types of repetitive bending, standing, floor exercises, and lifting up to 100 pounds. Position requires prolonged time standing; talking or hearing; and using hands to finger, handle, or feel. Requires extensive time walking; pushing/pulling; stooping, kneeling, crouching, or crawling; and, reaching with hands and arms. Requires normal range of hearing and vision. Must be able to hear alarms on equipment; client calls; instructions from physicians/department staff; and overhead pages over a load speaker. Must have close, distant, color, peripheral vision with depth perception and the ability to adjust focus as needed. This position requires the ability to work with repetitive motion actions of both hands with fine dexterity for prolonged periods of time. Also requires extensive repetitive motion actions of both feet; and both hands grasping simple/light. Travels and transports equipment/supplies to various communities and conducts program activities in rural communities within the Tuba City Service Area.
MENTAL:
Must be able to prioritize, use good judgment, and be able to coordinate a variety of projects while being frequently interrupted. Must have a non-judgmental attitude and be comfortable working with low-resource individuals/communities. Must demonstrate cultural sensitivity.
Environmental:
This job requires exposure to the following environmental conditions: Extensive exposure: work near moving mechanical parts, outdoor weather conditions, extreme cold (non-weather), extreme heat (non-weather); Occasional exposure: Wet/humid conditions (non-weather), fumes or airborne particles, toxic or caustic chemicals, risk of electrical shock. Typical noise level is loud.
Responsibilities:ESSENTIAL FUNCTIONS:
- Responsible for the coordination, implementation, and evaluation of community physical activity programs (Just Move It, Hiking Series, Get Fit Challenge, etc.) as outlined in SDPI goals and objectives.
- When appropriate, completes pre-and-post assessments of participants with establishing fitness goal plans, case management and maintains a database to track health outcomes.
- Arranges for necessary supplies, transportation, space, facilities, services, and equipment of community physical activity programs.
- Host monthly collaboration meetings to plan, organize, promote, and partner on community physical activity events with key stake holders.
- Collaborate and meet regularly with local public health agencies, tribal chapters, government agencies, community groups, health care providers, county agencies and other organizations to gather feedback on physical activity needs in their respective communities.
- Attend partnership meetings as needed to address the need to improve youth, adult and senior fitness health outcomes.
- Utilize appropriate media and marketing techniques in the development and implementation of adult & youth physical activity programs, collaborating with TCRHCC Public Relations when appropriate.
- Responsible for outcome reports and analysis of the community physical activity programs.
- Incorporate and integrate culturally appropriate concepts, modifications and activities when delivering health promotion/diabetes prevention education and services.
- Assist diabetes intervention programs (Take C.H.A.R.G.E. and Lifestyle Balance Program) as a Health Coach as needed.
- Assist with delivery of Group Fitness Classes.
- Assists HPDP Fitness Center operations such as registration; orientation/training; equipment maintenance; and, exercise room supervision.
- Assists with inventory and maintenance of equipment and program vehicles.
- Assists Physical Education Specialists with school Fitnessgram assessments and data collection/analysis.
- Supports culture of safety and take extra steps to ensure safety of all. Report injuries, safety concerns, and any emergency event as stated in TCRHCC policy.
- Demonstrate knowledge of emergency first aid procedures and administer emergency exercise related medical treatment as needed. Participates in mandatory in-services regarding Fire Safety, and OSHA. - Communicate effectively with supervisor, program staff, program partnerships, hospital staff, and community members. Communication abilities include written and verbal skills that demonstrate the ability to motivate or encourage others to alter lifestyles and group dynamics.
- Provide assistance to other departments within Community Health Division.
- Understands and adheres to established compliance, safety, and quality assurance standards.
- Ensure Proper PPE is always used while on duty. Face surgical mask is to be worn in all hospital areas. Proper handwashing is required. If water is not available use hand sanitizer. Social distancing will be practiced by department.
- Proper PPE is used inside assigned Clinics or Unit. NIOSH-approved N95 mask filtering face piece respirator or higher, is available, this included eye or face masks shield, gloves, and isolation gown.
- Complete all donning and doffing tasks in a safe acceptable method and discard of used PPE accordingly. Complete provided training.
- Performs other duties as assigned.
