We are seeking a licensed Physical Therapist to join our team at a skilled nursing facility. As the Physical Therapist, you will be responsible for evaluating and treating residents with physical impairments and functional limitations to help them achieve their highest level of mobility and independence.

Responsibilities:
- Evaluate residents' physical abilities, functional limitations, and mobility needs using standardized assessments
- Develop and implement individualized treatment plans based on assessment results, resident goals, and physician orders
- Provide skilled physical therapy interventions, including therapeutic exercise, manual therapy, and modalities
- Monitor and document resident progress and adjust treatment plans as necessary
- Communicate with interdisciplinary team members, including physicians, nurses, occupational therapists, and speech therapists, to coordinate care and ensure optimal outcomes for residents
- Educate residents and family members on physical therapy goals, treatment plans, and home exercise programs
- Maintain accurate and timely documentation of physical therapy services provided
- Participate in team meetings, in-service education, and quality improvement initiatives
- Maintain current knowledge of physical therapy practice and relevant regulations
Qualifications:
- Current license to practice as a Physical Therapist in the state of California
- Minimum of 2 years of experience in a healthcare setting, preferably in a skilled nursing facility
- Knowledge of current physical therapy practice standards and regulations
