What You'll Do
The Physical Therapist Assistant will play a critical role in executing Suvida's integrated healthcare model to strengthen the provider-patient relationship and engagement by enhancing the quality and affordability of care. This individual will focus on supporting the clinical staff and patients through a suite of physical therapy services and physical activity/ movement programs. The physical therapist assistant will perform routine physical therapy treatments and group classes according to treatment plans under the direction of a licensed physical therapist. Essential responsibilities consist of but are not all inclusive:
- Responsible for performing routine physical therapy treatments under the direction of a licensed physical therapist.
- Create home exercise programs for patients and provide appropriate education and documentation of programs.
- Complete thorough documentation of treatment visits and patient communication
- Responsible for coordinating and/ or teaching weekly group movement/ physical activity classes for patients (fall prevention classes, chronic care management, etc...)
- Provide virtual physical therapy and/ or house calls for high-risk patients if needed.
- Provide patient-centered education on chronic disease states including movement/ physical activity used for prevention and treatment.
- Collaborate with providers, interdisciplinary care teams, patients and/or caregivers to identify and develop patient-specific and individualized goals.
- Participate in quality improvement programs and initiatives driven by clinical needs.
- Responsible for utilizing available data to implement physical therapy initiatives, such as related HEDIS/Star measures.
- Ensure patients have a positive experience by providing an elevated level of service promptly, professionally, and empathetically.
What You'll bring - Education Requirements
- Graduate from a Commission on Accreditation in Physical Therapy Educated accredited physical education program.
- Active license by the Texas Board of Physical Therapy Examiners, or the ability to obtain one.
- Current BLS Certification
What You'll Bring - Experience Requirements
- 2+ years of physical therapist assistant experience preferred.
- Bilingual (English and Spanish) required.
- Experience leading group physical activity classes preferred.
- Geriatric specific experience preferred.
- Advanced Microsoft office platforms including word, excel, and power point.
Physical Demands
While performing the duties of this job, the employee is required to sit for prolonged periods of time, stand, walk, talk, hear, and reach with hands and arms. Hand eye coordination is necessary to operate computers and other office equipment; must be able to operate a motor vehicle. When representing Suvida Healthcare at external events, the Physical Therapist Assistant may also be required to stand for prolonged periods of time, kneel, crouch, bend and lift, and able to move up to 30 pounds.
