Qualifications:
- Licensed and Registered: Active PTA license and registration in New York.
- Experience: Demonstrated experience in adult PTA.
- Communication Skills: Strong ability to connect with both patients and office staff.
- Technical Proficiency: Familiarity with EMR software is beneficial.
- Homecare Experience: Previous experience in homecare is a plus.
Responsibilities:
- Patient Interaction: Communicate effectively with office staff and patients to ensure seamless care.
- Scheduling: Coordinate and schedule visits directly with your patients.
- Care Planning: Conduct patient visits and carry out the personalized care plan developed by the supervising OT, tailored to the patient's functional abilities and specific needs.
- Follow-up: Conduct follow-up visits as planned, ensuring quality and consistency in care.
- Documentation: Accurately document each visit in our web-based EMR system, adhering to Medicare and company guidelines.
- Compliance: Maintain active credentials and follow all HIPAA and company policies.
