Physical Therapist - Pavilion of Logan Square

To plan, develop, organize, implement, evaluate and direct the physical therapy services, as well as its programs and activities, in accordance with the current rules, regulations, guidelines and standards of practice for a skilled care facility. To confer with physicians and other practitioners to obtain additional resident information. To monitor and suggest revisions in treatment programs / plans as indicated. To integrate other therapy treatments and other aspects of the resident’s health care to develop an inclusive plan of care. To meet with administration including the administrator, medical director and director of nursing in planning and developing continued therapy services for the facility. Preforms administrative duties such as completing medical forms, reports, evaluations, studies, budgets etc., as necessary.

Physical Therapist Essential Duties and Responsibilities:

·       Review physicians’ referral, resident’s condition and medical record to determine resident’s qualification for therapy services

·       Administer evaluations, tests or examinations to resident to collect information to determine resident’s needs and therapy required

·       Explain physical therapy policies, procedures and programs to personnel, residents and family members as needed

·       Educate and train residents and family members concerning prescribed programs and activities.

·       Determine appropriate treatment, selecting activities and exercises based on medical and social history of resident

·       Participate in development and implementation of resident assessments, MDS and care plans including initial, annual and quarterly or as needed

·       Update and demonstrate procedures and changes to residents, resident’s representative and staff as indicated

·       Provide therapy in residents’ room as needed

·       Assure that a current file is initiated and maintained for each resident being treated including treatment records/minutes, progress notes, treatment plan orders and authorizations

·       Assist/prepare the resident for treatment and administer therapy in accordance with established policy and procedures

·       Provide necessary materials and equipment for residents to preform required therapy.

·       Teach basic living skills to residents as necessary

·       Completes documentation as required. Notes are informative and descriptive of the care provided and the resident’s response

·       Participates in discharge planning, development and implementation of the residents’ discharge plan of care and related assessments

·       Other duties as assigned

Physical Therapist Education and Experience Requirements:

• Degree in physical therapy from an accredited program
• Licensed to practice physical therapy in operating state
• Previous experience in long-term care facility is preferred
• Strong communication and multi-tasking skills
• Compassionate and empathetic personality

Physical Therapist Physical Demands:

·       Must be able to move intermittently throughout the workday

·       Must be able to push, pull move and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move and/or carry such weight a minimum distance of 50 feet

·       May be necessary to assist in the evacuation of residents during emergency situations

·       The noise level in the work environment is usually moderate
