Physical Therapist - Odell Medical Search

Rehab/Wellness Center on the West Coast of Oregon is seeking a Physical Therapist to join their 17 friendly therapists where they have a supportive and collaborative team who values integrity and best practice patient care.

They deliver rehabilitation services to a diverse outpatient caseload; they provide care to inpatients in a 21 bed Critical Access Hospital! They also have a Swing bed rehab program within the hospital, served by a dynamic interdisciplinary team.

They have a spacious remodeled gym furnished with newer equipment, including electric mat tables, powered parallel bars, and Sci-fit bikes. In addition they also have a dynamic vestibular program utilizing the RealEyes xDVR Video Goggles. Each therapist has their own computer-on-wheels to document at point of service.

About the role:

• Evaluates assigned patients and establishes a treatment plan and goals, using data from a variety of sources, such as patient records, interviews, observation, and team members.
• Provides patient treatment according to plan of care and modifies plan to meet patient needs, and updates plan as appropriate.
• Completes documentation accurately, timely, according to regulatory and state licensure requirements, and within professional standards.
• Collaborates with other healthcare professionals as necessary to ensure smooth continuous care of the patients. May act as a liaison with nursing, medical and other therapy staffs to facilitate problem solving and coordination of other services and act as an educational resource.
• May mentor and provide oversight to new caregivers, physical therapist assistants, and/or therapy aides. Provides clinical instruction and training to students.
• Recommends and participates in development of new services and programs, quality improvement and monitoring activities.

What you bring:

• Bachelor's, Master's or Doctorate Degree Required: Physical Therapy accredited by the Commission on Accreditation in Physical Therapy Education or closely related field (foreign equivalent accepted)
• Required: Physical Therapist license State of Oregon
• Required Upon Hire: Basic Life Support issued through the American Heart Association or American Red Cross
