Summary:
- As part of a vibrant team, participate in providing client related therapy services, including but not limited to, activities which assist clients in restoring or improving independent function relative to activities of daily living within the scope of practice to clients.
- Performs initial and ongoing client assessments and applies appropriate goals and interventions to resolve identified problems.
- Provides Physical therapy services to client according to physician orders and regulations.
- Assures physicians are notified in a timely manner of changes in client status and physician’s orders are obtained as needed for changes to the plan of care.
- Identifies and obtains/makes appropriate modifications to equipment/environment to meet client needs.
- Provides teaching and instruction to client/caregiver to promote physical and emotional health.
- Attends required staff meetings and case conferences.
- Contributes to the coordination of services through effective and timely communication with other members of the health care team.
- Maintains the caseload and agency productivity standards.
- Responds to referrals to Physical Therapy in a timely manner and documents care according to the agency standards.
- Maintains clients’ self-esteem, privacy and confidentiality of personal information.
- Assures client rights are protected and that all clients/caregivers are made aware of the client’s rights and responsibilities.
- Initiates emergency procedures as appropriate.
- Clinical and administrative documentation adheres to agency, professional, and regulatory guidelines for timeliness and content.
- Performs other duties within scope of practice, as assigned by Sr. Director of Clinical Operations

CERTIFICATES/LICENSES:
- Current unrestricted license as a Physical Therapist in the State of Colorado (required)
