• PHYSICAL THERAPIST HOME HEALTH JOB SUMMARY

  • Provides individualized physical therapy to Home Care patients. Delegates treatments to the appropriate support staff. Assesses patients and their home situation, communicates with physicians and other disciplines involved, provides care and education to patients and caregivers, and acts as a liaison to facilitate patient care needs.

• PHYSICAL THERAPIST HOME HEALTH JOB SPECIFICATIONS

  • Education and/or experience: Licensed as a physical therapist in the State of Tennessee with experience of 2 years preferred. Home care experience preferred.

    Knowledge: Working knowledge of Medicare, Medicaid, State of Tennessee and Joint Commission Standards preferred. Working knowledge of computers required.

    Skills/Abilities: Requires strong assessment and communication skills, organization and time management. Must be self-directed.
