Job Description:

As a valued member of our client's Home Health team, your role as a Physical Therapist will involve delivering top-quality Physical Therapy services to patients right in the comfort of their homes. You will also assess patients' functional capacity and devise treatment plans aimed at enhancing their physical and cognitive abilities, leading them toward an improved quality of life.

Required Qualifications:

To qualify for this role, you must hold a Master's degree or higher and possess a valid Physical Therapist license in Pennsylvania (PA). A Regular Driver's License (DL) is also necessary. Moreover, we require your experience in a Healthcare Setting to be more than a year.

Preferred Qualifications: 

Applicants who can provide their PT license number will be preferred.

Key Responsibilities:

Your responsibilities will encompass conducting Physical Therapy assessments and creating a custom treatment plan. You will also maintain records of patients' responses to the treatment and any notable changes in their conditions. Patient and caregiver education is another vital part of your role, alongside the supervision of a therapy assistant and certified nursing assistant as required. Lastly, your role will also involve you in preparing detailed reports and active participation in team development activities.

Schedule and Shift Details:

Your work schedule will be from Monday to Friday, ranging from 8:00a/9:00a-5:00p. You will also work one weekend per month (both Saturday & Sunday during day shifts with scheduled patients) and two holidays (one in summer and one in winter). There will be no on-call duty.

Travel:

The job involves local travel to patients' homes.
