This position involves delivering high-quality physical therapy services to patients within their homes, covering the Montoursville area and surrounding regions, including Lycoming & Clinton Counties. The role demands a collaborative approach to patient care, requiring frequent travel to residential homes and the use of advanced technology to ensure efficient service delivery.
Key Responsibilities:
- Develop and implement patient care plans in collaboration with the healthcare team.
- Provide physical therapy in home settings, ensuring patient safety and comfort.
- Manage documentation and charting using Electronic Medical Record (EMR) systems, including Practice Partner EMR.
- Coordinate care with other healthcare providers, maintaining a high standard of service.
- Effectively manage time and scheduling, ensuring timely patient visits and adherence to care plans.
Candidate Requirements:
We are searching for a highly motivated individual who meets the following criteria:
- Required:
- A Bachelor's or higher degree in Physical Therapy from an accredited institution.
- A valid Physical Therapist license in Pennsylvania (PA).
- At least one year of experience in a healthcare setting.
- A valid Driver's License (DL).
- Experience with Electronic Medical Records (EMR), including specific systems such as Practice Partner EMR.
- Preferred:
- Please provide your PT license number when applying.

Schedule and Shift Details:
- Monday to Friday, 8:00 AM - 5:00 PM
- Full-time, with 1 weekend/month and 2 holidays/year
Travel:
- Local travel within Lycoming & Clinton Counties, visiting residential homes.
