Our Award-Winning Client is actively seeking a skilled Home Health Physical Therapist to join their dedicated care team in Montoursville, Pennsylvania. Recognized for excellence and with a national presence, our client has earned numerous awards for their commitment to providing top-tier care.

This position involves delivering high-quality physical therapy services to patients within their homes, covering the Montoursville area and surrounding regions, including Lycoming & Clinton Counties. The role demands a collaborative approach to patient care, requiring frequent travel to residential homes and the use of advanced technology to ensure efficient service delivery.

Key Responsibilities:

• Develop and implement patient care plans in collaboration with the healthcare team.
• Provide physical therapy in home settings, ensuring patient safety and comfort.
• Manage documentation and charting using Electronic Medical Record (EMR) systems, including Practice Partner EMR.
• Coordinate care with other healthcare providers, maintaining a high standard of service.
• Effectively manage time and scheduling, ensuring timely patient visits and adherence to care plans.

Candidate Requirements:

We are searching for a highly motivated individual who meets the following criteria:

• Required:
  • A Bachelor's or higher degree in Physical Therapy from an accredited institution.
  • A valid Physical Therapist license in Pennsylvania (PA).
  • At least one year of experience in a healthcare setting.
  • A valid Driver's License (DL).
  • Experience with Electronic Medical Records (EMR), including specific systems such as Practice Partner EMR.
• Preferred:
  • Please provide your PT license number when applying.

Schedule and Shift Details:

• Monday to Friday, 8:00 AM - 5:00 PM
• Full-time, with 1 weekend/month and 2 holidays/year

Travel:

• Local travel within Lycoming & Clinton Counties, visiting residential homes.
