Key Responsibilities:

• Manage and deliver patient care plans.
• Collaborate effectively with the care team.
• Consistently meet critical deadlines for charting.
• Efficiently manage time and schedule.

Schedule and Shift Details:

• Monday to Friday; 8:00 AM/9:00 AM - 5:00 PM (Physical Therapy within this schedule).

Travel:

• Local travel to residential homes is required.

Qualifications:

• Bachelor's degree or higher.
• 1+ years of experience in healthcare.
• 1+ years of experience with Electronic Medical Records (EMR), including TruChart EHR or other EMR systems.
• Licensed Physical Therapist in Pennsylvania (New graduates or candidates with less experience are welcome).
