As a Physical Therapist in our client's Home Health team, you will be responsible for providing Physical Therapy services to patients in their homes, assessing patient functioning levels, and developing treatment programs to improve independence and physical/cognitive functioning. You will also be involved in patient and caregiver education, supervision, and reporting on patient progress.Key Responsibilities● Perform Physical Therapy assessments and develop treatment programs● Record and report patient's reaction to treatment and condition changes● Instruct patients and caregivers in therapy programs and device use● Supervise physical therapy assistant and, when appropriate, the certified nursing assistant● Prepare reports and participate in staff and Agency development activitiesSchedule and Shift Details● Schedule (Including any on call/weekends): Within M-F 8am-5pm; part time; 1 weekend/month; 2 holidays/yearTravel● Local travel to residential homes
