Premise Health is proud to partner with the University of Notre Dame to provide on-site healthcare to their employees and dependents. We are currently looking for a PRN Physical Therapist to join the amazing team at our employer health center located in Notre Dame, IN!
Will work as needed only for the following shifts: Monday 3:30-7pm, Wednesday 3:30-7pm, Friday 2-7pm and Saturdays 8-12pm
ESSENTIAL JOB FUNCTIONS
- Follows up with plan of care by modifying as needed and addressing any issues surrounding the ability to improve.
- Develops plans of care that may include: ROM/flexibility, strengthening, balance and coordination activities, core stability program and posture, utilization of manual skills, and modalities including US and ES heat/ice as needed.
- Establishes a comprehensive and functional HEP that the client can follow and understand.
- Ensures activities are appropriate and contribute towards decreasing pain/improving function in a reasonable timeframe.
- Establishes a connection with patients, significantly improving their satisfaction and compliance with the program.
- Speaks on health-related topics in your scope of practice to individuals and small groups.
- Works in coordination with the wellness director to assess potential clients in need of further assistance as well as referring patients to appropriate classes and programming.
- Administers physical therapy treatments by giving massages.
- Handles patients’ questions and concerns professionally and courteously.
- Documents patient care services by charting in patient and department records.
- Willingly adapts to new ideas and addresses the ever-changing work demands of team members.
- Maintains a clean, neat work environment and presents a professional image at all times.
- Treats all CareHere employees, customers, clients, vendors, and candidates with courtesy, respect and dignity.
- Maintains strict confidentiality related to company, client, employee and candidate information.
- Understands and follows safety and security procedures and practices preventative safety procedures as set forth by CareHere.
- Immediately reports all accidents and incidents to the immediate supervisor or Human Resources.
- Uses only equipment trained to use and operates all equipment in a safe manner.
- Travels by car, air or other means as necessary to complete or conduct company business.
- Fully understands and adheres to the systems provided by CareHere to control assets and expenditures.
- Adheres to all company policies, procedures and business ethics codes.
- Executes all terms and conditions as set forth in the CareHere Employee Handbook.
BASIC QUALIFICATIONS
Education:
- Doctor of Physical Therapy (DPT) degree from an accredited physical therapy
program
- Valid state-specific physical therapy license
- Nationally recognized certification in exercise science, personal training or similar exercise-related area
- Maintain current CPR certification for Health Care Providers
Knowledge and Skills:
- Thorough knowledge of physical therapy principles and practices
- Knowledge of CPT & ICD-9 coding procedures
- Strong organizational skills, time management skills, and the ability to work unsupervised
- Ability to travel domestically by car, air and other means with travel of up to 10%
- Computer literacy, including knowledge of basic software applications and familiarity with MS Office, electronic health records, internet, and mobile
