Physical Therapist, In House - PRN at Blue Palms Deland/Daytona Beach - Blue Palms Health and Rehabilitation Center of Deland

We are looking for a committed and patient-focused PRN Physical Therapist to join our in-house, therapy program at Blue Palms Health & Rehab located in Deland, FL.

You will offer effective and client-centered services that will enable people to confidently and independently carry out everyday tasks or activities..

Responsibilities:

• Evaluate, plan and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing
• Assure all treatment is delivered in accordance with an established plan of care.
• Provide clinical support and instruct patients, families, and caregivers.
• Monitor patient response to treatment intervention.
• Complete required forms and documentation in accordance with company policy and state/federal regulations

Qualifications:

• Must be a licensed Physical Therapist in the state where services are rendered.
• Long-term care experience preferred, or proven work experience as a Physical Therapist
