About the Job
Be a part of our M Health Fairview Rehabilitation Services team at our Elk River & Fridley clinics! We are seeking a physical therapist to provide pelvic health and orthopedic outpatient PT services. The Elk River and Fridley M Health Fairview Rehab Services teams partner to provide outpatient, multi-specialty therapy services to the northwest Twin Cities metro area. Preferred candidate will have experience in providing interventions for Women’s, Men’s, and Pediatric pelvic health populations. Where training and experience is limited, M Health Fairview offers mentoring and development opportunities for professional growth.
- FTE 1.0, authorized for 80 hours per pay period with opportunity for additional hours, if desired.
- Schedule: Four days per week, Tuesday - Friday, two days at each location.

M Health Fairview Rehabilitation offers a broad range of services that serve patients across ten acute-care hospitals, three post-acute settings and 70 outpatient adult and pediatric therapy clinics. Consisting of Physical, Occupational and Speech Therapy as well as Audiology and Cardiac & Pulmonary Rehab, our therapists collaborate with colleagues in all medical settings and offer dozens of specialty programs.

The Physical Therapist partners with the patient and their care providers while performing evaluation and re-evaluation. Captures and applies pertinent data, performs examination, and determines treatment diagnosis
Develops an individualized plan of care through collaboration with the patient and family
Analyzes data to identify needs and potential risks, settles prognosis for recovery, and establishes individualized goals
Implements and modifies care plan based on reassessment and patient response
Coordinates care and communicates effectively with interdisciplinary team for all aspects care & establishes appropriate discharge plan
Partners with patients and families to provide skilled intervention
Implements and alters treatment to reflect change in patient status and response to treatment
Assesses patient and/or caregiver learning needs and provides appropriate education
Documentation/Billing
Uses acceptable medical terminology and follows professional practice standards.

Qualifications:Required License/Certification/Registration
Physical Therapy registration/licensure in the state of Minnesota or temporary license in MN
Current Basic Life Support (BLS) certification
Preferred Education & Experience
Doctorate degree in Physical Therapy
1+ year of Physical Therapy practice
1+ year experience with Women’s, Men’s, and Pediatric pelvic health
