Circle of Care Pediatric Home Healthcare Company has an immediate position for a Physical Therapist to treat patients ranging in age from birth through age 20 in the home, daycare, or community setting as prescribed by the attending physician.
This opportunity can start as a PRN position and transition to a full-time salaried position if the candidate so chooses, with a very flexible schedule.
Qualifications for PT:
- Graduate of a physical therapy school approved by The American Physical Therapy Association --or-- The Council on Medical Education of Hospitals for the American Medical Association Licensed or registered by the State of Texas
- One (1) year of experience in a home health care setting preferred
- Pediatric experience preferred
Some of the Responsibilities include:
1. Prepares an assessment of the patient's condition and periodic evaluations and summaries, develops a plan of care and problem list, and writes clinical/progress notes on the date of the visit and incorporates the same in the patient's record.
2. Assists the physician in evaluating patient's by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests.
3. Instructs patients and their families in the use of prosthetic, orthotic, and assistive devices (canes, wheelchairs, etc.).
4. Supervises the physical therapy assistant, and evaluates the effectiveness of the services furnished by reviewing the assistant's notes.
5. Participates with staff, patient, and physician in discharge planning activities and completes the physical therapy discharge summary within the designated framework of the Agency policies.
Special Requirements:
- Must have reliable transportation
- Must have current auto insurance coverage
- Valid state driver's license
