We are seeking a dedicated and skilled Physical Therapist for our Home Health division to provide high-quality, patient-centered care in the home setting. This role is essential in fostering a positive rehabilitative environment for patients recovering from illness, injury, or surgeries. As a Physical Therapist in home health, you will evaluate each patient's physical condition, develop individualized treatment plans, and implement therapy interventions aimed at maximizing each patient’s independence and quality of life. You will engage closely with patients, families, and caregivers to educate them about their conditions and the necessary steps for recovery. This position demands effective communication skills, clinical expertise, and a compassionate approach as you work with individuals of varying needs and backgrounds. The successful candidate will possess a strong understanding of physical therapy practices and demonstrate the ability to adapt therapies to suit the home environment. • Graduation from an accredited Physical Therapy Program.

• 1 year of home health experience as a physical therapist

• Candidates must possess strong caring interpersonal skills
