Physical Therapist, PT - Cornerstone Rehab

We are hiring a Physical Therapist to deliver care to our patients and work with a diverse team of clinical professionals.

*Currently Hiring for Full-Time, Part-Time or Per Diem*

*Sign-On Bonus for Full-Time $5,000*

Responsibilities:

• Provide physical therapy to adult and geriatric patients.
• Prepare documentation, progress notes, and discharge summaries.
• Establish a plan of care in accordance with patients' needs and capabilities.

Qualifications:

• Valid State Physical Therapist License and Credentials
• SNF and/or Acute care experience
