JOB SUMMARY:
Evaluates and performs physical therapy treatments on patients with injuries, disabilities and diseases according to prescription of a physician. Maintains appropriate documentation for patient charts and third-party reimbursement agencies. Performs other record-keeping activity as required. The Registered Physical Therapist staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs, and to provide the care needed as described in the unit's/area's/department's policies and procedures.
DUTIES & RESPONSIBILITIES:
- Evaluates and performs Physical Therapy treatments on patients according
- to physicians' orders
- Formulates written treatment plans for patient which reflects the age and psychosocial development of the patient
- Effectively utilizes treatment modalities available in department
- Reassesses patients and modifies treatments as needed, documenting changes in patient chart. Consults with physicians when appropriate
- Accurately completes all documentation for patient charts according to department policy and procedures
- Prepare forms necessary for third-party reimbursement within 24 hours of treatment
- Communicates with physicians on patient progress as appropriate. Attends physician visits with patient upon request
- Provides instructions for home treatments and care to patients and care-givers
- Measures, orders and fits patients with various types of supports and braces according to physician orders
- Educates nursing staff of hospital and other facilities regarding use of modalities, range of motion, bed-positioning, gait training, as needed to augment patient care
- Instructs patients and staff on posture and body mechanics
- Assists with in-service training of nursing staff when requested
- Supervises patient treatment by Physical Therapy Assistants, re-evaluating every three weeks
- Communicates with PTA and physician through verbal and written exchange of patient status, progress and changes
- Follows Departmental Policies for documentation of medical record, billing and continuing education
- Incorporates discharge planning into treatment process in a timely manner
- Integrates patient/family education into treatment program
- Actively involved in Quality improvement in everyday practice and through peer review process
QUALIFICATIONS:
Education
Minimum of B.S. degree in Physical Therapy. Licensed as P.T. in New Hampshire and Vermont (if applicable), or hold a temporary license if awaiting examination for licensure.
Work Experience
Student affiliations or work experience in geriatrics, pediatrics. Orthopedics helpful.
