Key Responsibilities:

• Assist Physical Therapists in implementing individualized treatment plans for long-term care residents
• Provide therapeutic exercises and interventions to improve patient mobility, strength, and independence
• Monitor patient progress and communicate with the therapy team to adjust plans as needed
• Educate patients and caregivers on exercises, safety techniques, and rehabilitation strategies
• Foster a compassionate, supportive, and positive environment for patients and team members alike

What We’re Looking For:

• Active PTA license in the state of Wisconsin
• Experience in long-term care or geriatric rehabilitation is preferred, but not required
• Strong communication skills and a patient-centered approach to care
• Ability to work independently while collaborating effectively with other healthcare professionals
• Flexibility and a positive attitude

As a Physical Therapy Assistant at Evergreen, you will

• Implement Physical Therapists plan of treatment in Skilled Nursing Facility and Home Health
• Cover for full-time PTA's planned vacations and some weekends and holidays
