Job Details

• Location: Fallsburg, NY - Enjoy the serene beauty and community spirit of this charming area.
• Salary: $110,000 - $125,000 per year, depending on experience
• Employment Type: Permanent, Full-Time
• Schedule: Monday to Friday, 8 AM - 5 PM
• Patient Visits: 2 to 6 patients per day in their homes

Job Requirements

• Master’s Degree in Nursing or related field
• Valid Physician Assistant license in the state of New York
• 5+ years of progressive care experience
• BLS and ACLS certified
• Valid driver’s license and access to a vehicle for patient visits
• Strong computer skills, including proficiency in Word, Excel, and PowerPoint

Responsibilities

• Provide primary, urgent, and acute medical care to patients in their homes
• Conduct initial patient screenings and physical examinations
• Collaborate with the care team to develop and implement patient care plans
• Prescribe and manage medications and treatments
• Maintain accurate and current patient records
• Facilitate family meetings and provide educational resources
• Deliver palliative and end-of-life care as needed
