Following the principles of a Patient-Centered Medical Home, works as part of a multidisciplinary team to provide high quality and compassionate care. Provide comprehensive primary health care services to students on the Morningside Campus, with emphasis on illness prevention and health education. ResponsibilitiesOversees the care of a panel of primary care patients, attending to patients perceived problems by reviewing care received from other healthcare providers, laboratory data, secure messages from Medical Services providers, and from the patients themselves. Order tests as needed to ensure maintenance of health and appropriate management of chronic medical conditions.Delivers compassionate, culturally sensitive, and efficient patient care. Provide effective patient care through the evaluation, diagnosis, and treatment of acute, episodic care, and chronic illness and injury, follow-up of labs, radiological/other studies, and referrals. Provide direct clinical care with a significant emphasis on health maintenance and preventive measures. Attention to the patient's perceived problems should be balanced with good medical and psychosocial care of the whole person.Performs complete physical exams at a frequency appropriate to age and gender and orders appropriate laboratory, x-ray, tests, and other diagnostic and medical procedures to meet short-term health care needs, ensure maintenance of health, and appropriate management of chronic medical conditions.Manages the appropriate referral of patients to other health care providers for specialized care. Referrals are made by balancing patient needs and Medical Services guidance.Educates patients based on age, gender, psychosocial and cultural context, and overall health of the patient on illness or its complications, and on ways to maintain a healthy lifestyle. Advises the patient with an illness on the most clinically appropriate and effective management weighing and discussing the potential risks and benefits of treatment and embraces the principle of patient-centered care.Adhering to the Patient-Centered Medical Home model, works closely with other members of Medical Services team (nurse practitioners, registered nurses, clinical case managers, nutritionists, medical assistants, and health services assistants). Serves as a consultant for nurse practitioners and other team members and clinicians to ensure seamless handoffs to care by other providers, both internal and external to Medical Services. Shares responsibility for compliance with AAAHC ambulatory accreditation requirements. Participates in an on-call schedule. On-call responsibilities include answering calls directed by the first vendor, FoneMed. Participates in Medical Services continuous improvement and quality management efforts by knowing and understanding Medical Services strategic goals, and by participating in team huddles and projects to achieve said goals. Participates in clinical department meetings, interdepartmental committees, working groups, and other efforts to solve problems, and proposing ideas to improve quality of care.Understands and complies with public health initiatives. Maintains written and verbal confidentiality of all patient and department information at all times, following the Columbia Health confidentiality and electronic mail policies without exception. Ensure timely EMR documentation according to Columbia Health and department policy.May participate in teaching medical students and postgraduate residents.Practice travel medicine.Promotes a positive image of Medical Services by demonstrating knowledge and understanding of patient needs.Professional and intellectual growth through continuing medical education (CME) and the ongoing pursuit of medical/academic interests is essential to maintaining a high standard of practice. Columbia Health will provide weekly on-site CME of relevance to the current scopes of service. Additionally, all provider(s) are afforded resources and time for extra-curricular CME activities. Providers will attend CME courses and stay abreast of medical literature.Exercises broad professional responsibility for medical practice and functions in a peer collaborative relationship with other clinical disciplines. Operates in accordance with licensure and certification in accordance with the guidelines and standards of Columbia University, Columbia Health, the American College Health Association, of professional societies, national, state, and local departments of health.
