- DEFINITION OF POSITION
- The Hospitalist is accountable for:
- Admitting all patients of the primary care physicians (“PCP”) as referred to the Hospital.
- Identifying themselves as the hospitalist physician admitting for the PCP while the patient is in the Hospital.
- Giving their business card to the patient and/or family members.
- Explaining to the patient and/or family members that the patient’s PCP will be informed of the care being rendered during the course of the patient’s admission.
- Reviewing the admission for medical necessity.
- Complying with the Hospital Pharmacy Formulary.
- Complying with the Health Plan’s and PCP’s specialty delivery network.
- Adhering to all clinical best practice core measure quality indicators.
- Adhering to Hospital Clinical Documentation Plan.
- Seeing patients in the ER in a timely fashion, 24 hours a day, 7 days per week, 365 days per week, as medically necessary.
- Responding to the ER calls within (30) minutes of such call.
- Arranging for specialty consults to be provided to patients in a timely manner.
- Conducting daily clinical review and medical management of Hospital patients.
- Attend daily multidisciplinary rounds.
- Establishing accurate diagnoses.
- Educating providers on the hospitalist program.
- Communicating with the patient’s PCP throughout the hospitalization and or LTAC or SNF stay when needed, especially upon admission and discharge of the patient.
- Dictating discharge communications within 24 hours of the patient’s discharge.
- Communicating with the receiving physician upon transferring the patient to another facility, including SNF and LTACH.
- Attending and actively participating in Medical Staff, Medical Staff committee and Departmental meetings.
- Expediting diagnosis, treatment and discharge planning of patients.
- Communicating with the Care Managers and Medical Directors.
- Returning all pages promptly in particular to the PCP’s.
- Maintaining an effective productive working relationship with the Hospital’s clinical and Administrative personnel.
- Calling PCP prior to certain invasive procedures, including cardiac cath, etc.
- Calling PCP on patients with long stays (>5days) on day 5 and every 2 days thereafter for updates.
- Maintain established protocol when transferring care of patients to other hospitalists for scheduled days off.
- Rounding on patients as often necessary to facilitate appropriate utilization.
- Daily morning clinical meetings to review patient status with Lead Hospitalist, the Chief Medical Officer, Unit Managers and/or Case Managers.
- Participating in Hospital Quality Initiatives.
- Participate in other documentation programs required by Hospital, including, but not limited to, any training sessions required to implement and maintain such systems.
