Rate Caps for Submission:
Malpractice Must be included in rates.
Orientation half the hourly/callback rate*** kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Hourly ***-240 - Per hour, not OT
Holiday Must be 1.5x the regular rates Per holiday hour, no OT
Schedule/Availability Requirements:
ASAP- as soon as credentialed - 6 months to a year
Shift times: Weekdays: 8:00 am to 5:00 pm, with a 1 hour lunch
Frequency: Monday Friday, every week- no call, would also consider someone who could do only a few days/week
Facility Location: 1595 Soquel, Suite 400, Santa Cruz, CA
Setting: Internal Medicine- will consider a strong FP but that is not ideal.
FTE: 1.0

Duties: Typical IM clinic population average age 70. About half male/female with multiple comorbidities, such as diabetes, arthritis, heart disease, chf, etc
Required Procedures: N/A
EMR: Cerner
Patients per Day: Other providers seeing 12-18 a day depending upon complexity (includes new patients)
Support Staff: MA's, MOR's, APP (one in clinic) and RN assigned to clinic
Reason for Coverage: Provider Retiring
Deal Breakers? Not board certified
