I. Purpose of Position
Provide comprehensive primary care services for Access patients in a collaborative effort with all care team members.

III. Essential Job Functions/Duties
- Perform, order, interpret and/or analyze patient histories, medical records, various medical reports and examination information to diagnose patients' condition and choose treatment options.
- Prescribe or administer treatment, therapy, medication, vaccination, and other medical care to treat or prevent illness, disease, or injury in compliance with medical standards, laws and regulations.
- Patient care may include hospital rounds for some medical patients, prenatal care in the clinic and obstetrical services in the hospital.
- Collect, record and maintain patient information within the electronic medical record.
- Monitor patients' conditions and progress and reevaluate treatments as necessary.
- Explain procedures and discuss test results or prescribed treatments with patients.
- Advise patients concerning diet, activity, and hygiene and disease prevention.
- Refer patients to medical specialists or other practitioners when necessary.
- Maintain shared goals, mutual respect and trust with all care members to include nurses, pharmacists, psychologists, and other health care providers.
- Direct and coordinate patient care activities of supporting medical staff.
- Keep up-to-date technically and apply new knowledge to the job.
- Provide medical guidance and expert advice to management or other groups within Access.
- Active and constructive participation in care team development and process/quality improvement projects.
- Effective communication with all care team members based on ‘Provider Expectations’ outlined by the Medical Director.
- Coordinate patient care across multiple settings. Work collaboratively community partners to provide quality patient care.
- Works effectively on a team and communicate respectfully with patients and all staff based on shared goals and mutual respect. This includes demonstrating adaptability to continuous efforts to improve patient care and willingness to be an active care team member engaged in ongoing process and quality improvement projects within the framework of the Patient-Centered Medical Home.
- Honors patients’ right to privacy and confidentiality. Adheres to strict patient, customer and business confidentiality standards.
IV. Additional Job Functions/Duties
- Participate in process improvement and quality improvement projects outlined by the Chief Medical Director and Site Medical Director.

VI. Education, Experience and Credentialing
Required Qualifications:
- Licensed Medical Doctor or Doctor of Osteopathic in Wisconsin or eligible for license in Wisconsin.
- Board certified (or board eligible) in Family Practice with or without Obstetrics.
- Insurable for liability coverage as a physician.
- Evidence of continuing education, as required.
- Experience with supervising, motivating and directing medical staff.
Preferred Qualifications:
- Experience working with mid-level providers.
- Experience working with diverse patient population or working within a community health center.
- Fluency in English and Spanish
