Work Setting: Hospital Dayton, OH
Scope of Work: Cover inpatient EP consults and EP cases
Licenses, Certifications, Requirements: OH license ( Must have) DEA w/OH address Board certified ACLS BLS Covid-19 vaccination card Diploma Internship/Residency Certificate TB Test (within last 12 months) Flu vaccination card (within last 12 months) Procedure logs (past 24 months showing inpatient encounters) 3 peer references (within the past 12 months) CAQH COI for all affiliations Min 1 year experience
EMR: EPIC Haiku (Secure Chat - MANDATORY for all providers to download to their phones)
