Board Certification
Michigan license
BLS
Provider must be local to Michigan
Must have clean background for quick credentialing
Must be available for above dates
PM&R required procedures: pre-admission screening review and documentation per CMS Guidelines, Prescribing Narcotics, Discharge Summary/Med Rec, Weekly Care Plan Documentation, Post admission evals, face to face visits, IPOCS, PAPE/H&P
Inpatient Rehab Unit (20 beds)
Will not cover outpatient clinic only Hospital Inpatient Rehab coverage
100% adult rehab
Number of Patients Per Day: 10-20 on the unit
24 hour Lab available
24 hour Anesthesia Department
Documentation System/EMR: Cerner
Trauma Level/Designation: Level III
Certification Requirement: BLS
