Physician - Vascular Surgeon - Loyal Source

Loyal Source Government Services is looking for a Physician to work at an Outpatient Clinic in North Chicago, IL.

Job Summary: 

physicians shall provide comprehensive clinical Vascular-Thoracic Surgical Services, including but not limited to the following: clinics, on-call services, and surgical procedures including but not limited to Thoracic Procedures such as: Video-Assisted Thoracoscopic Surgery (VATS), Lobectomy, thoracotomy, diaphragm plication, chest wall hernia repair, decortication, pleurectomy, chest tube management w/tPA, along with other complex Thoracic surgery procedures and Vascular Surgical procedures, including but not limited to: dialysis access (AV Fistulas, AV Grafts, and Perm-a-Cath placements), extremity vascular procedures, and other vascular surgical procedures as indicated.

Experience of surgeons who will be providing services under this contract including a copy of their CV/resume, BLS Certification, and proof of Board Certification/ Eligibility by the American Board (certificate, website verification, or letter indicating passage of CORE examination) in one or more of the following specialties: Thoracic Surgery with noted vascular training, Vascular Surgery, or Cardiothoracic/Cardiovascular Surgery. All certifications shall be current. Additionally, indicate if a surgeon has had any malpractice claims, termination or suspension of privileges, or has been asked to be removed from contract service, and if so, provide information on the circumstances surrounding and the outcome. Also indicate if the surgeon has clinical privileges at a community medical facility near the FHCC.

Pay: $296/hr - In Clinic

$56.24/hr - On Call
