The ARCHES Project for Polk County Rapid Rehousing would like a Case Manager to join their team! This position will be responsible for providing case management and rental assistance to chronically homeless individuals and families in Polk or Marion County through Community Action's Rapid Re-Housing (RRH) program. This includes eligibility screening, program enrollment, and participant follow-ups and check-ins. Case Managers will also be responsible for processing required paperwork, data entry and other general program supports. This position will directly align with the Housing First initiatives of the ARCHES Project.

MINIMUM QUALIFICATIONS The requirements listed below are representative of the knowledge, skill, and/or ability required.
EDUCATION and/or EXPERIENCE
A High School Diploma or GED.
Two years of related social service experience; or an
Equivalent combination of education and experience may be considered.
Ideal candidates will possess two or more years of homeless service delivery experience.
Preference will be given to applicants with knowledge of/or experience with: the principles of coordinated assessment, working with unsheltered or vulnerable populations, as well as best practices in homeless service delivery.
CERTIFICATES, LICENSES, REGISTRATIONS
Driver qualifications are required for this position. Valid driver license, insured vehicle available for use on the job, and a satisfactory drive record for the previous three years are required.
Candidate must pass a comprehensive MWVCAA background screening prior to employment.
Basic Life Support/First Aid Certification is required within first 30 days of hire.
Candidate must pass pre-employment and random drug/alcohol screenings.
KNOWLEDGE, SKILLS, AND ABILITIES
Ideal candidates will possess two or more years of homeless service delivery experience.
Preference will be given to applicants with knowledge of/or experience with: the principles of coordinated assessment, working with unsheltered or vulnerable populations, as well as best practices in homeless service delivery.
Basic proficiencies in computers, MS Office products, and database software and web tools.
Must possess excellent planning, organization and time management skills.
Ability to effectively communicate both orally and in written form.
**ESSENTIAL DUTIES AND RESPONSIBILITIES This job description is not intended to be all-inclusive. Employee will also perform other reasonably related business duties as assigned by immediate supervisor and other management as required.
Facilitates the distribution of rapid re-housing and barrier removal funds. Ensures access to housing assessments and resources for clients.
Assesses client eligibility and aids in gathering documentation as required for intake, enrollment, and financial assistance.
Provides direct service to participants, including case management and housing placement support to assist participants in becoming self-sufficient and overcoming barriers to accessing permanent housing.
Develops action plans that identify participant needs, barriers, and strengths.
Participates in regular follow-up meetings and check-ins with clients.

WORK ENVIRONMENT
Regularly works indoor and outdoor in cold weather, heat and other extreme climate events.
Close quarters, often with a public client population experiencing homelessness, substance use disorders, and/or severe and persistent mental illness, and/or unpredictable behavior.
Frequent interruptions and demands; frequent noise.
This position is not eligible for remote work.
Ability to work outside of normal business hours, including nights, weekends, and holidays.
