About Healthcare Network
Healthcare Network is a Federally Qualified Health Center serving Collier County, Florida. We provide patient-centered medical and dental services to all age groups, regardless of insurance status or ability to pay. We offer the highest level of coordinated care across the spectrum from medical or dental visits, to integrated behavioral health care and on-site lab, pharmacy, and radiology services.
Through partnership with Volunteer Florida, AmeriCorps, and Centers for Disease Control (CDC), Healthcare Network has received a grant to expand care services through concentrated outreach efforts of AmeriCorps members. As an AmeriCorps member, you fuel the engine for change by joining the army of difference-makers who positively impact lives across Collier County.
About Volunteer Florida/AmeriCorps Program
Volunteer Florida administers the AmeriCorps program across the state. AmeriCorps is a national service program that provides opportunities for individuals to serve their communities in many different capacities. In exchange for a living stipend, educational award and valuable on-the-job experience, members work in nonprofit organizations, educational foundations, and government agencies for one year, helping improve the health of our community, one patient at a time.
AmeriCorps members are selected and managed by their host organization and spend a year addressing issues critical to the organization's mission. Healthcare Network's grant specifically targets the areas of Immokalee and Golden Gate City in Collier County, which are historically underserved communities. This grant will ensure that access to health and dental care is given to those who need it most with emphasis on ensuring that they receive the care they need in the most comfortable fashion possible.

THIS IS A 100% GRANT FUNDED POSITION
POSITION OVERVIEW:
The Population Health AmeriCorps will support activities related to Healthcare Network's Public Health AmeriCorps program through targeted intervention. Specifically, AmeriCorps members will be serving within the clinics to ensure that all patients are connected to the available services in Collier County in languages other than English (traditionally Spanish, though Healthcare Network also offers Creole translators). Under guidance of the Director of Population Health, the Associate Director of Care Management, Population Health Provider Champions, and the Quality Control Analyst members will serve the patient to close clinical gaps in care. This is a 100% grant-funded position.
KEY RESPONSIBILITIES:
In an effort to promote equity, diversity and inclusion, AmeriCorps Community Referral Members will serve closely alongside a diverse group of Healthcare Network Community team members to recognize differences amongst our population and celebrate the contributions each culture brings to our organization and community. With open minds and hearts, members will execute the following responsibilities:
- Support the program in Collier County, with emphasis on the underserved populations to alleviate any potential healthcare disparities exist
- Assist is cultivating strong relationships between patients and healthcare providers
- Provide access to care by sharing the benefit of health care by ensuring that patients are scheduled and seeing their providers so that they will be able to receive essential care
- Educate patients with identified chronic conditions on the importance of lifestyle adjustments, such as, diet, exercise, and other changes.
- Assist patients in check in, check out, registration, and informing them of clinical resources that are available to them
Training
- Participates in AmeriCorps and Healthcare Network training, as required per program guidelines.
QUALIFICATIONS:
- High school graduate, or GED is required
- Experience working or volunteering in a community relations position
- Experience with data collection
- Bilingual (English and Spanish or Creole) to meet the needs of our community
- Flexible team player
- Ability to use a computer, email or data collection tools
- Ability to make phone calls and schedule appointments

POSITION SERVICE ENVIRONMENT: You may need the following abilities to perform the essential functions of this position:
