Population Health Medical Assistant - Squirrel Hill Health Center

Squirrel Hill Health Center, a Federally Qualified Health Center (FQHC), is hiring a Population Health Medical Assistant.

The Population Medical Assistant (MA) will support the Quality and Population Services Team by contacting patients to schedule appointments, relaying messages and laboratory results from providers, preparing for and actively participating in huddles with the clinical team, working on quality initiatives, and carrying out such other MA duties as SHHC may need.  

After successfully completing the training period, this position has the potential for a Hybrid work schedule. He/she will be required to work in all SHHC locations as needed.

ESSENTIAL DUTIES:

• Appointments: call patients to schedule appointments for follow up, immunizations, or in house testing.
• Visit Pre-planning: before the day of the appointment.
  • Review patient’s chart and the physician's notes from the patient's last visit as well as notes from other clinicians who delivered interval care.
  • Ensure that the patient completed all tests ordered at last visit. If not, the MA calls the patient to remind them and schedule the labs before the upcoming visit.
• Huddles:
  • Print daily huddle.
  • Highlight key patient data.
  • Share information obtained during pre-planning with MA assigned to daily huddles with assigned provider.
  • Request orders for immunizations or screenings identified as open gaps in care.
• Quality:
  • Outreach patients to educate on importance of preventive screenings.
  • Mail/fax orders, fitkits, or other information needed to complete screenings.
  • Assist patients with scheduling appointments (i.e. HbA1c).
• Inbox: managePopulation Team inbox.
  • Reviews lab results with patients under the direction of Physician/Provider, relaying the Physician/Provider message as stated on the task.
  • Directs patients’ clinical questions to RN and/or Physician/Provider.
  • Addresses refill requests per protocol.
  • Accurately and consistently documents all patient interactions in Electronic Health Record.

QUALIFICATIONS:

1. Minimum 3-5 years of experience as a Medical Assistant.
2. Certification as a Medical Assistant preferred.
3. Ability to understand and support the goals of population health management.
4. Skill in Electronic Health Record and ability to learn new systems.
5. Medical terminology, ICD-10 and CPT coding knowledge.
6. CPR certified.
7. Knowledge of a second language preferred.
8. Must be committed to serving populations consistent with SHHC’s mission.
9. All CDC recommended vaccinations, including annual Covid and Flu, required.
