About the Job
Curative is seeking a driven and organized individual to join our Population Health Care Navigation team in a Program Coordinator capacity. In this role you will have the opportunity to collaborate with leaders and experts to establish priorities, organize the execution of the Population Health Strategy, drive overall scheduling processes for Baseline Visits and assist the Care Navigation leadership with evaluating process effectiveness and implementing enhancements. The ideal candidate will have experience in developing, documenting and driving Health Care Navigation type programs in a health plan setting. This position will report directly to the VP, Population Health Care Navigation.
Duties and Responsibilities
Manage and coordinate the scheduling for baseline visits between Care Navigation team and the Clinical team
Assist the Care Navigation leadership team with identifying and driving enhancements to the program
Handle multiple tasks and special projects at a time
Prioritize work with little to no direction
Collaborate with direct leader and stakeholders to communicate and present population health and care navigation initiatives to various audiences
Identify and document areas of opportunity with care navigation processes
Participate in meetings and conference calls to identify areas of opportunity, documentation and follow up of action items
Manage and track department initiatives
Perform other duties as assigned
Qualifications
Previous experience in developing and enhancing population health and/or care navigation programs
Demonstrated ability to manage multiple tasks and initiatives at one time
Exceptional executive level communication skills and attention to detail
Demonstrated ability to troubleshoot and determine when issues/risks require the assistance of others
Analytical and proactive
Previous experience in health plans
Education and/or Experience
3+ years of experience in case management/care navigation/health plan/medical office role
At least 2 years of experience working in a clinical setting
Strong ability to use and learn new technology tools
Understanding of medical terminology
Experience working in healthcare, hospitality or customer service.
Associate's degree (A.A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
