The PPS Coordinator Rehab is responsible for planning, developing and overall coordination of activities that support the Prospective Payment System (PPS) process for each rehabilitation inpatient unit. Responsibilities include conducting audits to ensure documentation is consistent with Medicare assessment data, educating RN and therapy team members on quality indicators, and collecting, auditing, and analyzing outcomes data. Plans and implements quality improvements for key nationally and regionally ranked indicators and reviews records for regulatory compliance.
Responsibilities:A day in the life of a PPS Coordinator Rehab at Hackensack Meridian Health includes:
1. Accurately completes and transmits the IRF-PAI (Inpatient Rehabilitation Facility-Patient Assessment Instrument) for each rehabilitation inpatient.
2. Collects pertinent demographic and clinical information and assigns appropriate quality indicator data on the IRF-PAI.
3. Performs clinical chart reviews and collaborates with physicians and interdisciplinary team members to ensure accurate assessment and documentation for each patient.
4. Communicates with coders and physicians to determine appropriate etiologic diagnosis, impairment group code, and comorbid conditions.
5. Provides ongoing staff education to ensure compliance with CMS (Center for Medicare and Medicaid Services) guidelines.
6. Compiles data for monthly reports, PI (Performance Improvement) Dashboards, and regulatory tracking.
7. Data Analysis: runs monthly reports on multiple outcome measures and CMS compliance; performs data analysis and develops action plans to ensure optimal patient outcomes with targets to exceed regional benchmarks.
8. Develops, conducts or revises Performance Improvement activities.
9. Reviews records for compliance to regulatory requirements.
10. Other duties and/or projects as assigned.
11. Adheres to HMH Organizational competencies and standards of behavior.
Education, Knowledge, Skills and Abilities Required:
1. Bachelor's Degree.
Education, Knowledge, Skills and Abilities Preferred:
1. Inpatient Rehab experience.
Licenses and Certifications Required:
1. NJ State Professional Registered Nurse License, Physical Therapist License, Occupational Therapist License or Speech Language Pathology License.
