Primary Function of Position

As the Preferred Communities Case Manager, you will be responsible for implementing and following up on needs assessments for newly arriving medically vulnerable refugees. Your role involves determining the appropriate course of action, whether providing internal assistance or connecting clients to external sources of treatment. You will also cultivate and maintain partnerships with community agencies to ensure collaborative efforts are used in delivering the most effective treatment possible.

Major Position Responsibilities

1. Intensive Case Management: Manage intensive case management for medically vulnerable cases.

1. Progress Updates: Provide progress updates and collaborate frequently with referral agencies

1. Appointment Coordination: Coordinate extensive medical appointments.

1. Disability Applications: Apply for social security disability and appeals. Collaborate with community resources for the attainment of SSI for clients.

1. ADA Support: Collaborate with community resources for ADA housing, medical equipment procurement, and ADA transportation.

1. Resource Referrals: Refer medically vulnerable individuals needing social, interpersonal, emotional, or vocational resources when needed.

1. Case Records: Maintain case records and documentation on services provided.

1. Medical Liaison Collaboration: Collaborate and coordinate with medical liaisons as needed.

1. Service Provider Collaboration: Collaborate with other refugee service providers.

1. Professional Representation: Represent the agency in the community and workplace professionally and ethically.

1. Microsoft Office: Utilize basic knowledge of Microsoft Office applications.

1. Interpretation and Translation: Provide interpretation and translation services, as necessary.

1. Other duties as assigned.

Work Environment

1. This position is in a professional office environment using standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.

1. Some local travel may be required.

Physical Demands

1. This is a sedentary role; however, some filing is required. This would require the ability to lift files, open filing cabinets, and bend or stand, as necessary.

1. This role includes driving locally.

Education, Training, and Experience

1. A Bachelor of Social Work, Public Health, or Health Sciences or equivalent experience.

1. Experience working with medically vulnerable individuals.

1. Experience working with clients from a variety of cultures.

Background Screening Requirement

1. Must pass a national BCI/FBI background check.

Licenses, certifications, and credentials

1. Current Utah driver's license, proof of auto insurance with minimum policy liability limits of $50,000 per person and $100,000 per incident, 21 years of age or older, a clean driving record, and ability to meet CCS vehicle safety requirements.

Position status and expected schedule

1. This position is scheduled to work 40 hours per week, Monday through Friday, between the hours of 8:30 AM - 5 PM. Occasionally working hours may need to be adjusted to accommodate a night arrival.
