About the Program and Position:
The Preventive Case Planning Program provides support and coordinated treatment provision for children and families to work through high-risk family-identified concerns, learn new skills, and prevent the removal of children. Preventive Case Planners will partner with families directly to ensure children, youth, and families’ needs are met safely. Case Planners facilitate family discussions, achieve consensus on areas of growth, and support action planning to celebrate lasting change. How you get to use your skills…
Engage children, youth, and families in case planning activities to prevent out of home placement/support youth returning home from placement
Conduct assessments of strengths and needs
Implement Evidence Based Models of Solution Based Casework and Motivational Interviewing
Develop and carry out service plans to support youth and families in goal achievement
Act as an agent of change for high risk families
Assist children, youth, and families in achieving timely and lasting permanency
Collaborate with stakeholders and community partners
Schedule: Non-Exempt
Scheduled hours are 9:00 am - 5:00 pm; however, flexibility is required to meet department and agency needs.
Rotating on call schedule required
Keyword: Preventive Case Planner
Required Experience:
Bachelor’s Degree and 2 years of experience in the Child Welfare or Juvenile Justice systems
Experience in the human services field and working with at-risk populations, preferred
Ability and willingness to work with families in an 'in-home' context
Ability to understand child development
Excellent communication and writing skills
Excellent organizational skills
Ability to work effectively and patiently with others in difficult and/or stressful situations
Physical Requirements:
Ability to drive and operate a motor vehicle with passengers for at least four consecutive hours
Ability to participate in physical activities with program youth, including running, jumping, bending, and twisting
Ability to sit or stand for prolonged periods of time
Ability to walk up and down stairs repeatedly
