About the Job
CommuniCare Health Services, in conjunction with our medical practice, Personalized Health Partners, is currently recruiting for a Primary Care Physician.
THE ROLE
The Primary Care Physician (PCP) assists with implementation of strategic network initiatives, supports growth of our innovative healthcare practice, and provides high quality, value-based primary care to the senior patient population within our network.
Our network consists of a variety of payors, an accountable care organization (ACO), telemedicine, and Medicare advantage products. The PCP is instrumental by leading our person-centered care model in delivering quality care with smaller panels and more frequent touches. The physician will assist in streamlining healthcare collaboration of the interdisciplinary team while providing leadership in driving high quality clinical outcomes. This approach will reduce hospital and emergency room department admissions and visits, focus on preventive medicine and chronic disease management, and lead a culture focused on patient experience and satisfaction.
EDUCATION and QUALIFICATIONS:
Required:
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) graduate
- Internal Medicine or Family Medicine Board Certified or board eligible
- Active, unrestricted state license
- US work authorization
- 2+ years in primary care or family medicine
- Ability to work in a flexible environment and schedule
- Passionate about providing healthcare to the senior and low-income populations
Preferred:
- Fellowship training in Geriatrics
- Experience practicing medicine among Geriatric populations
- Experience or training working in a metrics, results driven healthcare model
- Be open to professional development through training, obtaining certifications if necessary, and attending team meetings
- Experience working in value-based care models
- Experience working in a collaborative healthcare setting to drive positive outcomes and achieve goals
- Training or experience in outpatient primary care setting
