Job Overview:
We are seeking a dedicated Primary Care Physician to join a comprehensive program focused on delivering holistic care to seniors, allowing them to live independently in their communities. This role involves working closely with an interdisciplinary team (IDT) to provide comprehensive assessments, develop personalized care plans, and oversee participant care across various settings, including nursing homes and hospitals.

Key Responsibilities:

• Perform in-person assessments and collaborate with the IDT to create individualized care plans.
• Oversee participant care, including admissions and discharges, and ensure effective coordination with medical specialists.
• Provide support and direction to Nurse Practitioners and other team members.
• Participate actively in IDT meetings, communicate participant changes, and ensure continuity of care.
• Conduct regular visits to nursing homes based on participant needs.
• Participate in a rotating schedule for night and weekend on-call duties.

• Master’s Degree / Doctorate Degree (MD or DO) from an accredited institution.
• Board Certification in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics preferred.
• Active California Physician License without restrictions.
• State CDS Certification and active DEA Registration.
• 1-2 years of clinical experience, preferably with geriatric patients.
