Job Description
The Stroke Coordinator serves primarily as an informative role with the interdisciplinary team, the public and hospital patients. This includes oversight of identifying stroke patients from admission throughout the continuum of care and ensuring appropriate education materials about their disease process are available to staff. Monitors the EMR for performance measure compliance, provides follow up calls to the patients post discharge and coordinating stroke outreach programs within the hospital and community settings. This consists of helping educate the public and staff on the importance of stroke prevention, what risk factors often contribute to strokes and what steps you can take to live a healthier life to prevent not just the onset of a stroke, but other health issues in general. The Stroke Coordinator also provides oversight of patients' care needs that have suffered a stroke to help them live a healthier lifestyle to prevent future additional strokes and continue their post-stroke care.
Responsibilities
SPECIFIC DUTIES AND RESPONSIBILITIES
Oversight of patients' transition from inpatient to outpatient including phone support to discharged patients and facilitating follow up care with physicians as needed.
Facilitate referrals to area resources, Stroke Support Groups as needed.
Education and consultation to other units for Primary Stroke Program.
Facilitate database management and informative retrieval as requested.
Function as a patient and staff resource for questions and concerns.
Assists with preparation and maintenance of Joint Commission Certification.
Assists with monitoring, maintaining, and addressing performance measure compliance.
Assists with monitoring and maintaining order sets and discharge instructions and compliance with Clinical Practice Guidelines.
Assists with maintaining education materials for staff, patients, and community.
Assist with facilitating the Primary Stroke Program Steering/QAPI Committee.
Perform other duties as required or directed.
Qualifications
KNOWLEDGE AND SKILL: Telemetry experience recommended; Stroke experience strongly recommended.

EDUCATION: Graduate of an accredited school of nursing. BSN or equivalent required. MSN, or CNL preferred.
YEARS OF EXPERIENCE: 2 years of Stroke experience highly recommended.
REQUIRED SKILLS AND KNOWLEDGE: The RN effectively communicates with peers, utilizes appropriate channels of communication, and maintains absolute confidentiality. The RN maintains competence and demonstrates evidence of continuing professional growth. The RN demonstrates the ability to accept and implement change and the ability to work in a culturally diverse setting.
LICENSES & CERTIFICATIONS: The RN holds a current licensure to practice nursing in Ohio. Membership in professional organizations is required. BLS certification is required prior to patient contact. Certification in area of specialty recommended
