PRN Case Manager - Agape Comfort Care Solutions

Agape Comfort Care Solutions is seeking a dedicated and compassionate Registered Nurse (RN) to join our team as a PRN Case Manager. In this role, you will play a vital part in delivering high-quality, client-centered care by overseeing service plans, conducting client assessments, performing supervisory visits, and maintaining detailed documentation to meet Georgia state home care regulations.

As a PRN Case Manager, you will be responsible for collaborating with clients, families, and healthcare providers to ensure our clients receive the best care possible. Your expertise in clinical assessment and commitment to quality care will support our mission of enhancing the quality of life for each individual we serve.

Responsibilities:

• Conduct Initial Assessments: Perform thorough client assessments, gathering comprehensive information about medical history, current needs, and goals to develop customized care plans.
• Oversee Service Plans: Regularly review and update client service plans to align with their evolving needs, coordinating with the care team to ensure consistent, client-centered care.
• Supervisory Visits: Conduct supervisory visits to client homes to evaluate the quality of care, ensure compliance with the care plan, and provide guidance as needed.
• Documentation and Compliance: Maintain accurate and timely documentation for all client interactions, care plans, and reports, adhering to Georgia state regulations for home care.
• Communication and Collaboration: Communicate effectively with clients, families, caregivers, and the care team to foster a supportive, coordinated approach to care.
• Quality Assurance: Participate in quality assurance activities to uphold high standards of care and continuous improvement.
• Additional Duties: Perform additional tasks as directed by the Clinical Director to support client care and company operations.

Qualifications:

• Licensure: Current, unrestricted RN license in Georgia.
• Experience: Minimum of 2 years of clinical experience in home health or a related field, with preferred experience serving geriatric populations or individuals with chronic conditions.
• Knowledge: Thorough understanding of Georgia state regulations for home care, and familiarity with medical terminology and medications commonly used by seniors.
• Skills: Excellent clinical assessment, communication, and interpersonal skills, proficiency in EHR systems, and strong organizational abilities.
• Other Requirements: Valid driver’s license, reliable transportation, and a willingness to travel to client homes within the service area.
