Job Title: Nurse Practitioner (NP) - Wellness and Health Assessments
Role Summary: We are looking for a dedicated and compassionate Nurse Practitioner to conduct comprehensive wellness visits and health assessments for a diverse population with chronic health conditions. This role is pivotal in delivering educational health opportunities, assisting in the reduction of care disparities, and enhancing the overall quality of care delivery.
Key Responsibilities:
- Conduct annual wellness visits and health assessments for patients with chronic conditions.
- Provide patient education to promote health and well-being.
- Collaborate with healthcare teams to close gaps in quality care, such as screenings and lab work.
- Contribute positively to team efforts to enhance care delivery.
- Travel within a 55-mile radius to provide patient care.
- Commit to a minimum of 30 hours per month of service.
Qualifications:
- Possession of a valid and unencumbered NP license in the state of practice.
- Experience in in-home risk assessments is preferred.
- Open to all NP specialties, excluding pediatrics.
- New graduates are welcome to apply.
